Abstract Purpose:To describe a case of choroidal detachment after PRESERFLO® MicroShunt(PMS) implantation.

Case report:A 76-year-old man underwent PMS surgery in his right eye for primary open angle glaucoma(POAG). Three days later, hypotony occurred in his right eye. He was treated with 1% atropine eye drops twice a day and bandage soft contact lens. Choroidal detachment was found at another eye clinic after 20 days. He was referred to our department. His visual acuity was right(0.06), left(0.7) at his first visit to our department. Intraocular pressure was 10 mmHg in his right eye and 16 mmHg in his left eye. Fundoscopic examination showed circumferential choroidal detachment in the right eye.

Results:Surgery treatment was carried out by inserting a 9-0 nylon monofilament suture from the distal end of the PMS reaching the opposite end of the tube. Choroidal detachment promptly disappeared in the next day. His visual acuity in the right eye recovered to(0.8).

Conclusions:Insertion of a 9-0 nylon suture is an effective and minimally invasive way for the management of hypotony and choroidal detachment after PMS. treatment.
