A 61-year-old male presented with sudden bilat-eral visual loss. Besides markedly reduced visual acuity, we observed bilateral homonymous hemianopia, normal fundus and normal pupillary reactions. Computed tomography (CT) and mag-netic resonance imaging (MRI) demonstrated mul-tiple cerebral infarctions involving both occipital lobes.

A 62-year-old female with operated stomach cancer developed leukoencephalopathy after long-term systemic treatment with carmofur. The visual acuity was almost no light perception with normal fundus and normal pupillary reactions. We detected Anton's syndrome. CT and MRI showed severe damage in the white matter in the bilateral oc-cipital lobes due to leukoencephalopathy.

The findings illustrate that cortical blindness is to be suspected in cases of sudden and severe bilateral visual impairment with normal fundus and normal pupillary reactions.
