Abstract Purpose:To describe the circadian rhythm of intraocular pressure(IOP)using the Triggerfish contact lens sensor(CLS)in a case of primary open angle glaucoma(POAG)after treatment with ripasudil 0.4% eyedrop instillation(add-on).

Case:A 40-year-old man with POAG was treated with omidenepag isopropyl 0.002% and dorzolamide 2%/timolol 0.5%. The circadian fluctuations in IOP were measured with CLS before and after instillation of ripasudil 0.4% eyedrops(add-on)q12H for 2 weeks. The circadian fluctuation(the difference between the average sleep time IOP mVeq and wake time IOP mVeq)represented by an IOP of 110.039 mVeq before ripasudil instillation and 115.493 mVeq after it. The amplitude of the cosine fit curve was 123.19 mVeq before ripasudil and 156.94 mVeq after it.

Conclusion:The IOP circadian fluctuation did not decrease on ripasudil instillation. This may be because ripasudil 0.4% eyedrops effectively reduce the IOP during both day and night. Further research of the effect of ripasudil on IOP circadian fluctuation is necessary.
