Abstract Purpose: To report the incidence and severity of conjunctival hyperemia after instillation of ripasudil ophthalmic solution in adult volunteers.

Cases and Method: This study was made on 40 eyes of 40 volunteers who were employees in an eye hospital. The series comprised 27 males and 13 females. The age ranged from 20 to 61 years, average 34 years. One drop of 0.4% ripasudil ophthalmic solution was instilled in each case. Conjunctival hyperemia was evaluated before, 15 minutes, one hour and 2 hours after instillation based on photograph of anterior segment. Intraocular pressure was measured using an applanation tonometer.

Results: Conjunctival hyperemia was evaluated as absent in 39 eyes and mild in one eye before instillation, as mild in 23 eyes and moderate in 17 eyes 15 minutes later, as absent in 21 eyes and mild in 19 eyes one hour later, and absent in 37 eyes and mild in 3 eyes two hours later.

Conclusion: Conjunctival hyperemia developed promptly after instillation of one drop of ripasudil ophthalmic solution. It was mild to moderate 15 hours after instillation and almost resolved 2 hours after instillation.
