要約　目的：遠見での同側性複視を主訴とする加齢関連遠見内斜視に対して，片側の前転法あるいは後転法を実施した。その手術結果を検討し，本内斜視の特徴を考察する。

対象と方法：初回手術として片眼の内直筋後転法あるいは外直筋切除前転法を行った，13名の加齢関連遠見内斜視患者。手術前後の斜視検査結果などを後ろ向きに検討した。

結果：手術時の平均年齢は74歳（59〜85歳）。後転法は5名に，前転法は8名に実施した。遠見内斜視角は術前平均9.9Δ（プリズムジオプトリー）（範囲：3〜18Δ），術後平均−0.2Δ（−3〜2Δ）であった。単位手術量（mm）当たりの矯正効果は，外直筋前転法が内直筋後転法よりもわずかに大きいと推測された。

結論：加齢関連遠見内斜視には，外直筋前転法も内直筋後転法もともに有効である。しかし，後転法では手術量を通常より増大することを考慮すべきである。

Abstract　Purpose：Patients with age-related distance esotropia was treated with unilateral resection of the lateral rectus muscle（RLM）or recession of the medial rectus muscle（RMM）. The clinical characteristics of this strabismus are discussed after analyzing the results of these surgeries.

Subjects and methods：Unilateral RLM or RMM were carried out as the initial surgery on 13 age-related distance esotropic patients. Results of examinations of strabismus were retrospectively collected before and after the surgery.

Results：The average age at surgery was 74（range 59-85）years. Five patients underwent recession, while 8 patients underwent resection. The angle of esotropia at distance was 9.9Δ（3-18Δ）and −0.2Δ（−3-2Δ）, before and after the surgery. The corrected angle per each surgical dose（mm）was slightly larger for RLM than that for RMM.

Conclusion：Both surgeries were effective to correct age-related distance esotropia. However, increasing the surgical dose should be considered when recession of the medial rectus muscle is selected.
