ドライアイ患者の自覚症状の季節変化および重症度に関する検討

Study of seasonal changes and severity of subjective symptoms of dry eye patients

要約　目的：ドライアイ患者の自覚症状の季節変動と重症度に関連する項目を後ろ向きに検討した。

対象と方法：2017年3月〜2019年2月に当院を受診したドライアイ患者を対象とした。質問表はDry Eye related Quality of life Score（DEQS）を使用し自覚症状を検討した。

結果：検討対象症例は，全体で延べ593名。DEQSの結果は自覚症状の強さを示すサマリースコアでは秋に最大，冬に最小であったが季節によって有意差はなかった。設問1「目がゴロゴロする（異物感）」，設問3「目が痛い」を自覚する割合は季節により有意差があったが，他の設問では有意差はなかった。症状を自覚する割合が最も少なかったのは，設問14「目の症状のため外出を控えがち」であった。設問14の症状を自覚する患者のサマリースコアは，この症状を自覚しない患者よりも有意に高く，点眼治療を行っても自覚症状が改善しにくい傾向であった。

結論：ドライアイの自覚症状は，季節間で有意差はなかった。設問14の自覚症状を有する患者は治療に難渋する傾向があり，従来の点眼治療だけでなく中枢神経系の異常や全身的な疾患も考慮し治療にあたる必要があると考えられる。設問14の自覚症状は，ドライアイの治療方針を考える1つの指標になると考えられる。

Abstract　Purpose：We retrospectively examined items related to seasonal changes and severity of subjective symptoms of dry eye patients.

Methods：To investigate the subjective symptoms of dry eye in different seasons, using the Dry Eye Related Quality of Life Score（DEQS）Questionnaire between March 2017 and February 2019.

Results：The summary score indicating the level of subjective symptoms was the largest in autumn and the smallest in winter, but there was no significant difference among the seasons. Questions 1 and 3 were significantly different among the seasons, but the other questions were not significantly different. The least common symptom was “tends to avoid going out of the house because of eye symptoms”（Q14）. The summary score for patients with a positive response to Q14 was significantly higher than that of patients with a negative response to Q14, and the former patients tended to be less likely to experience improvement in their subjective symptoms, even with eye drop treatment.

Conclusions：There was no significant difference in subjective dry eye symptoms among the seasons. Patients with dry eye with a positive response to Q14 tended to have difficulty in treatment. Therefore, it is necessary to consider not only eye drop treatment but also central nervous system abnormality and systemic disease.

Q14 of DEQS Questionnaire seems to be an index that considers the treatment plan for dry eye.
