Abstract Purpose:To report the usefulness of Follow-Up(FU)mode in optical coherence tomography(OCT)for detection of segmentation error(SE)in a glaucoma case.

Case:In 2008, a 55-year-old man was diagnosed with normal tension glaucoma and treatment was started. From 2012, we performed follow up observation of glaucoma by monitoring macular ganglion cell complex(m-GCC)thickness with NIDEK RS3000 and from 2017 with RS3000 advance.

Clinical Course:No SE was found on the baseline B scan at the initial OCT examination. The two sectors of glaucoma(G)chart(upper half circle/lower half circle)in his right eye measured 66/73 μm. The m-GCC thickness was initially unchanged, but when the FU test was introduced in 2017, the m-GCC thickness was increased from the baseline. The visual field did not change during this period. As the involvement of SE was suspected, the segmentation of the retinal layer was examined. At the time that the increase was noted, SE of the inner plexiform layer/inner nuclear layer line was found.

Conclusion:Awareness of SE is important in OCT examination, because SE misassesses glaucoma-related changes. It is important to check for SE at each inspection, but if it is not possible at every inspection, it should be verified that there is no SE at the baseline inspection. It is necessary to be aware of sudden changes in m-GCC thickness in the FU mode. In particular, when an increased thickness is recognized, it is necessary to suspect SE. The FU mode is useful for searching for SE.
