Abstract. Purpose:To report a case of allergic granulomatous angiitis who developed anterior ischemic optic neuropathy(AION)followed by bilateral central retinal artery occlusion(CRAO). Case:A 62-year-old woman presented with impaired vision in her right eye. She had developed bronchial asthma followed by treatment for 6 months. She had been diagnosed with allergic granulomatous angiitis and been receiving systemic corticosteroid since one month before. Findings:Corrected visual acuity was 0.1 right and 0.8 left. The right eye showed pale and edematous optic disc with central scotoma. The findings were compatible with the diagnosis of AION. She noticed impaired vision in the left eye a few days later. Visual acuity was reduced to 0.1. The left eye was diagnosed with CRAO. The right eye developed CRAO 4 days later. Systemic treatments including gamma globulin and plasma exchange were futile, resulting in visual acuity of zero in the right eye and 0.1 left 8 months after onset of ocular manifestations. Conclusion:AION and CRAO in the present case may have been causatively related with allergic granulomatous angiitis.
