漿液性網膜剝離を伴う網膜細動脈瘤に対する炭酸脱水酵素阻害薬の治療成績

Outcome of peroral carbonic acid anhydrase inhibitor for retinal macroaneurysm with serous retinal detachment

要約　目的：炭酸脱水酵素阻害薬の網膜細動脈瘤に伴う漿液性網膜剝離（SRD）に対する効果につき検討したので報告する。

対象と方法：2008年4月〜2019年3月に当院を受診した網膜細動脈瘤患者のうち，炭酸脱水酵素阻害薬内服のみで3週間以上経過観察可能であった，SRDを伴う連続症例5例（男性2例，女性3例）5眼（平均年齢74.2±10.9歳）につき，後ろ向きに内服前後の視力，中心窩網膜厚を比較した。

結果：SRDはすべての症例で消失し，消失までの期間は平均3.2±0.8か月であった。視力はlogMAR値で内服前0.32±0.07，SRD消失時0.04±0.09と有意に改善していた（p＜0.01）。中心窩網膜厚は内服前469.2±111.7μm，SRD消失時182.4±19.7μmと有意に減少していた（p＜0.01）。

結論：炭酸脱水酵素阻害薬は網膜細動脈瘤に伴うSRDの改善効果，視力改善効果があることが示唆された。

Abstract　Purpose：To report five cases of retinal macroaneurysm with serous retinal detachment（SRD）treated by oral acetazolamide.

Cases and Method：This retrospective study was made on 5 eyes of 5 cases who had macroaneurysm with SRD. The series comprised 2 male and 3 female eyes. The age ranged from 64 to 89 years, averaged 74 years. All the eyes were treated by oral acetazolamide more than 3 weeks. Visual acuity was evaluated as logMAR.

Results：Visual acuity averaged 0.32 before treatment and 0.04 when SRD disappeared. The difference was significant（p＜0.01）. Foveal thickness was measured by optical coherence tomography. It averaged 469μm before treatment and 182μm when SRD disappeared. The difference was significant（p＜0.01）.

Conclusion：Oral acetazolamide was followed by improved visual acuity and reduced serous retinal detachment in eyes with macroaneurysm.
