Abstract Background:Relapsing polychondritis is characterized by chronic recurrent inflammation involving multiple organs. Here we present a case of recurrent polychondritis diagnosed through the observation of serous retinal detachment.

Case:An 84-year-old woman was referred to our hospital with sudden vision loss in the right eye. Her visual acuity was limited to light perception in the right eye. The intraocular pressure was 7 mmHg in the right eye and 16 mmHg in the left eye. Scleral hyperemia and thinning of the corneal periphery were noted in the right eye. Both eyes exhibited a deep anterior chamber, while the right eye displayed 2+ inflammatory cells and flare. The right crystalline lens exhibited marked nuclear sclerosis and posterior capsule opacities. Ultrasound B-mode revealed retinal detachment and choroidal thickening.

 On a facial examination, saddle nose was observed, prompting a systemic review for suspected recurrent polychondritis. Simple chest computed tomography demonstrated tracheal collapse during expiration. An otolaryngology evaluation, specifically nasopharyngeal fiberscopy, revealed a nasal septum perforation. Upon meeting three of the diagnostic criteria established by Damiani et al., the patient was diagnosed with recurrent polychondritis. The patient underwent treatment with steroid pulse therapy and adalimumab.

Conclusions:Our findings underscore the importance of considering recurrent polychondritis in the differential diagnosis of scleritis associated with serous retinal detachment. Furthermore, they emphasize the significance of thorough facial and auricular visual examinations in diagnosing scleritis.
