Sulfur hexafluoride（SF6）ガス注入により改善したValsalva網膜症の1例

Intravitreal gas injection for the treatment of Valsalva retinopathy

要約　目的：Sulfur hexafluoride（以下，SF₆）ガス硝子体内注入により改善したValsalva網膜症の1例を報告する。症例：45歳女性。前日からの左眼光視症と飛蚊症を主訴に当科を受診した。発症前日にカラオケをした。所見：初診時視力は右眼（1.5），左眼（0.07）。左眼眼底にニボーを形成した網膜前出血と硝子体出血を認めた。発症4日後に左眼SF₆ガス硝子体内注入による血腫移動術を施行した。出血は徐々に吸収，拡散され，視力は術後1週で（0.3），10週で（1.2）に改善した。結論：Valsalva網膜症に対し，SF₆ガス硝子体内注入は有用であった。

Abstract.　Purpose：To report a case of Valsalva retinopathy treated with intravitreal sulfur hexafluoride（SF₆）gas injection. Case：A 45-year-old female presented with photopsia and floater in her left eye after karaoke singing. Findings：Corrected visual acuity was 0.07 in the left eye and 1.5 in the right eye. The left eye showed vitreous hemorrhage and preretinal hemorrhage of 4-5 disc diameter with niveau. Intravitreal pure SF₆ gas injection was performed. The mass of hemorrhage diffused and vanished 1 week later. The remaining hemorrhage under the internal limiting membrane disappeared 10 weeks later. Corrected visual acuity improved to 1.2. Conclusion：Intravitreal SF₆ gas injection was effective on Valsalva retinopathy with vitreous hemorrhage, and Karaoke singing was a possible cause of Valsalva retinopathy.
