下垂体後葉炎による動眼神経単独麻痺を発症した11歳男児例

Oculomotor nerve palsy in an 11-year-old boy with infandibuloneurohypophysitis

要約　目的：リンパ球性漏斗下垂体後葉炎（LINH）により動眼神経単独麻痺を生じた小児の1例を報告する。

症例：11歳，男児。頭痛および複視を主訴に当院眼科を紹介され受診した。右動眼神経麻痺と診断し，頭蓋内疾患精査のために頭部MRIを施行した。下垂体に造影剤にて増強される9.9mm大の病変を認めた。当院小児科の入院精査結果は，尿検査で中枢性尿崩症の所見であった。第7病日より中枢性尿崩症に対してデスモプレシン120μg/日の内服治療を開始した。第43病日に眼球運動制限は改善し，MRIで下垂体病変は9.1mmまで縮小した。動眼神経麻痺は自然軽快し，下垂体病変部も縮小傾向であることから，LINHの臨床診断となった。

結論：小児の動眼神経麻痺の原因として，頻度は少ないがLINHは留意すべきである。診断に際して，下垂体疾患では尿崩症などの内分泌異常が生じるため，眼症状や画像所見以外の病歴聴取も重要と考えられた。

Abstract　Purpose：To report an 11-year-old boy who developed oculomotor nerve palsy secondary to lymphocytic infandibuloneurohypophysitis（LINH）.

Case：An 11-year-old boy presented with headache and diplopia. He was diagnosed as right oculomotor nerve palsy. Magnetic resonance imaging（MRI）showed a lesion 9.9 mm in size in the pituitary gland with enhanced contrast. Urinalysis showed central diabetes insipidus. He started receiving desmopressin at the daily dosis of 120 μg. The eye movement improved and the lesion of the pituitary gland reduced to 9.1 mm in size 6 weeks later. Thereafter, the oculomotor nerve palsy improved and the pituitary gland continued shrinking. The clinical course led to the diagnosis of LINH.

Conclusion：The present case illustrates that LINH may manifest as oculomotor nerve palsy in a child.
