Abstract Purpose:To report an unusual case of cancer-associated retinopathy secondary to papi-llary thyroid carcinoma.

Case:A 64-year-old male was referred to us under the tentative diagnosis of retinitis pigmentosa. He had noted bilateral visual loss and night blindness since 3 years before.

Findings and Clinical Course:Corrected visual acuity was 0.15 right and 0.02 left. Both eyes showed narrowing of retinal vessels and diffuse atrophy of retinal pigment epithelium. Inflammatory cells were present in the vitreous. Kinetic perimetry showed severely constricted visual field in both eyes. Electroretinogram was extinguished. PET-CT imaging showed abnormal accumulation of 18F-FDG in the right lobe of the thyroid, suggesting malignant tumor of the thyroid gland. Surgical resection of the thyroid led to the diagnosis of papillary thyroid carcinoma. The visual functions slightly improved duringone year after surgery.

Conclusion:This case illustrates that papillary thyroid carcinoma may gradually progress without manifesting clinical symptoms and that it may induce cancer-associated retinopathy that simulates retinitis pigmentosa.
