要約 目的:経強膜的マイクロパルス毛様体光凝固術(MP-TSCPC)を緑内障患者に施行し,その術後成績を報告する。
対象と方法:対象はMP-TSCPCを施行し術後6か月以上経過観察ができた45例50眼(67.5±13.4歳)で,原因疾患は原発開放隅角緑内障(38眼),落屑緑内障(5眼),血管新生緑内障(3眼),続発緑内障(3眼)および高眼圧症(1眼)であった。術前後の眼圧,視力,点眼・内服スコアならびに合併症についてレトロスペクティブに調査した。
結果:20眼(40%)では術後6か月以内に眼圧コントロール不良のため追加手術を施行し(追加群),30眼(60%)では追加手術を行わなかった(非追加群)。非追加群の術前,術後1週,1,3および6か月の眼圧は,それぞれ20.3±4.3,13.4±4.6,15.5±4.6,16.9±4.1および18.2±6.0mmHgであり,術前に比較して術後1か月および3か月で眼圧が有意に低下した(p<0.05)。視力,点眼・内服スコアは術後に変化しなかった。追加群では非追加群に比較して術前眼圧が高く(p<0.0005),緑内障手術の既往が多かった(p<0.005)。術後合併症として,瞳孔の軽度散大を41眼(82%)に認めたが,術後6か月でほぼ消失した。
結論:MP-TSCPCによって約半数の症例で眼圧下降が得られた。しかし,その眼圧下降効果は短期間であった。術前の眼圧と緑内障手術歴は適応症例を選択するうえで,重要な基準となる可能性がある。
Abstract Purpose:To evaluate the postoperative outcomes of micropulse transscleral cyclophotocoagulation(MP-TSCPC)in patients with glaucoma.
Subjects and Method:We included 50 eyes of 45 patients(67.5±13.4 years of age)who underwent MP-TSCPC and were followed up for at least 6 months after surgery. The causative diseases were primary open-angle glaucoma(38 eyes), exfoliation glaucoma(5 eyes), neovascular glaucoma(3 eyes), secondary glaucoma(3 eyes), and ocular hypertension(1 eye). We measured pre- and postoperative intraocular pressures(IOP), visual acuity(VA), medication scores, and complications.
Results:Twenty eyes(40%)underwent additional surgery due to poor IOP control within 6 months after surgery(additional group), and 30 eyes(60%)did not undergo additional surgery(non-additional group). In the non-additional group, the average IOP was 20.3±4.3, 13.4±4.6, 15.5±4.6, 16.9±4.1, and 18.2±6.0 mmHg preoperatively and at 1 week, 1, 3, and 6 months postoperatively. The IOP significantly decreased at 1 and 3 months postoperatively(p<0.05). VA and the medication scores did not change postoperatively. The preoperative IOP was higher in the additional group than in the non-additional group(p<0.0005), and more eyes had undergone previous glaucoma surgery(p<0.005). Postoperative complications included mild dilation of pupils in 41 eyes(82%), which resolved almost completely 6 months after surgery.
Conclusions:In approximately half of the patients, MP-TSCPC resulted in only short-term IOP reduction. Preoperative IOP and history of glaucoma surgery may be important criteria in selecting patients for MP-TSCPC.
