網膜蔦状血管腫合併の網膜静脈閉塞症に伴う黄斑浮腫にアフリベルセプトが著効した症例

Intravitreal aflibercept was effective for macular edema in a case of retinal racemose hemangioma with retinal vein occlusion

要約　目的：網膜蔦状血管腫（RRH）は先天性，非遺伝性で通常無症候性の治療を要しない疾患であると考えられている。今回，筆者らは両眼性の視神経乳頭上のRRHに合併した網膜静脈分枝閉塞症に伴う黄斑浮腫に対し，アフリベルセプトにて加療した経験を報告する。

症例：48歳，男性。1週間前からの急な左眼視力低下を主訴に受診。初診時の視力は右（1.2）左（0.1），両眼眼底には視神経乳頭上の血管奇形と増殖組織，左眼は視神経直下に1乳頭径の境界不鮮明な白色隆起物およびその周辺に網膜刷毛状出血や軟性白斑，漿液性網膜剝離を伴う黄斑浮腫を認めた。蛍光眼底検査にて下方の網膜静脈分枝閉塞症を認めた。直ちにアフリベルセプト硝子体内注射を施行したところ，治療後1か月目に浮腫が著明に軽快し，視力は左（0.9）と改善，白色隆起物は境界鮮明となり縮小した。さらに治療後3か月目には視力は左（1.2）に改善した。現在治療後1年経過し，黄斑浮腫の再発を認めず，経過良好である。

結論：RRHは，無症候性で治療を要しない疾患であると考えられているが，これに伴う静脈閉塞は動静脈の直接吻合によって，静脈内に高い圧がかかり，静脈壁が変則的に肥厚，血管内皮がダメージを受けることで，血栓ができやすくなることが原因と考えられる。抗血管内皮増殖因子薬を投与することで，血管をいったん収縮させ，静脈への負荷が軽減したことも治療が奏効した要因でないかと考える。

Abstract　Purpose：To report a case of retinal racemose hemangioma with retinal vein occlusion and macular edema that responded to intravitreal injection of aflibercept.

Case：A 48-year-old male presented with sudden visual loss in the left eye since one week before.

Findings and Clinical Course：Corrected visual acuity was 1.2 in the right eye and 0.1 in the left eye.Both eyes showed vascular malformation with proliferative tissue in the optic disc. The left eye showed brush-like hemorrhage, soft exudates and serous retinal detachment inferior to the disc. Fluorescein angiography showed findings compatible with branch retinal vein occlusion（BRVO）. The left eye was treated by intravitreal aflibercept. Macular edema subsided with visual acuity of 0.9 one month later. Visual acuity improved to 1.2 another 2 month later. There has no recurrence for one year until present.

Conclusion：Retinal racemose hemangioma is generally considered as benign without need for treatment. The present case shows that BRVO may develop due to elevated venous pressure and may induce macular edema.
