要約　目的：ポータブル電動トレパンで摘出した強角膜片を用いた角膜移植のドナー持込菌曝露の現状と術後経過の報告。対象と方法：2007年8月～2010年5月の期間に角膜移植を行った46眼のドナー強角膜片，保存液の細菌培養，レシピエント臨床経過を調べた。結果：ドナー強角膜片は10.9％，保存液は6.5％で培養陽性で，検出菌はいずれもStaphylococcus aureus-MRSAを含むグラム陽性球菌であった。ドナーからの持込感染はなかった。持込菌の有無でレシピエント術後予後の差はなかった。結論：ドナー持込感染はなかったが，強角膜片細菌培養陽性であれば眼内炎のリスクが上がるとされているため，今後も十分な注意を要する。グラム陽性球菌については保存液や洗浄液の見直しが必要である。

Abstract.　Purpose：To report the state of bacterial contamination of donor cornea obtained by a portable electric trephining and the clinical course after keratoplasty. Cases and Method：This study was made on 46 donor eyes obtained from the Nagasaki Eye Bank during 34 months through 2010. Bacterial culture was made from the preservation media and scrapings of rim of donor eye after removal of cornea using a portable electric trephining. Results：Microorganisms were cultured in 5 out of 46 specimens（10.9％）of scrapings and in 3 specimens（6.5％）of preservation media. They were all gram-positive cocci including Staphylococcus aureus（MRSA）. No patient in the present series developed endophthalmitis after keratoplasty. There was no difference in the clinical course between patients with and without bacterial contamination. Conclusion：Use of electric corneal trephining may induce bacterial contamination of donor eyes. Caution is needed regarding the preservation media or cleansing fluid for gram-positive cocci.
