要約 目的:網膜の微小塞栓症の光干渉断層計アンギオグラフィ(OCTA)とマイクロペリメータによる所見の報告。
症例:37歳女性が,未破裂の右内頸動脈瘤に対し瘤内塞栓術を受けた。術翌日から右眼の視野中央部に微小な欠損を自覚し,術2日目に当科を紹介され受診した。
所見と経過:矯正視力は右1.2,左1.5で,右眼の中心窩耳下部に白色の病変があった。OCTAで血流欠損があり,マイクロペリメータでその部位に感度低下があった。これらの異常は4か月後に自然寛解した。
結論:眼底の微小な検出と経過観察に,OCTAとマイクロペリメータが有用であった。
Abstract Purpose:To report a case of retinal microembolism evaluated by microperimeter and optical coherence tomography angiography(OCTA).
Case:A 37-year-old woman was referred to us for a subtle defect in the center of her right visual field. She had received embolization for unruptured aneurysm in the right internal carotid artery 2 days before.
Findings and Clinical Course:Corrected visual acuity was 1.2 right and 1.5 left. The right fundus showed a small white spot inferior temporal to the fovea. OCTA showed defective blood flow in the spot. Microperimeter showed impaired sensitivity in the area. The findings normalized spontaneously 4 months later.
Conclusion:OCTA and microperimeter were useful in the detection and follow-up of small lesions in the fundus.
