要約　目的：涙囊内結石の処理に16ゲージ（G）シースが有用であった1症例を報告する。

症例：62歳男性。数か月前から右の流涙と眼脂があった。

所見：通水試験で通水可能であったが，膿性貯留物の逆流があった。鼻涙管狭窄を疑い，涙道内視鏡下でのシース誘導シリコーンチューブ挿入術を予定した。術中，涙囊内に大きな結石を発見した。18Gシースで結石を搔爬して下鼻道への排出を試みたが，結石は固くて大きいため搔爬は困難であった。16Gシースを用いると，効率よく搔爬することができた。下鼻道に排出された結石は長径11mmであった。

結論：16Gシースはサイズが大きく，物体を押す力が強いことから大きな結石を効率よく搔爬することができた。

Abstract　Purpose: To report effectiveness of 16 gauge sheath for treatment of dacryolith in the lacrimal sac.

Case: A 62-year-old male with epiphora and discharge of the right eye was referred to us. Forced irrigation test showed passage of the fluid into the nose and backflow of putrid mass. We suspected nasolacrimal duct stenosis and scheduled sheath guided intubation. Intraoperatively, we found a large dacryolith in the lacrimal sac. The hard large dacryolith made it difficult to discharge the dacryolith into the inferior nasal meatus using 18 G sheath. However, 16 G sheath could effectively discharge the dacryolith, which was 11 mm in the longitudinal diameter.

Conclusion: Sixteen G sheath is large enough in size and is able to push a material strongly into the inferior nasal meatus, and therefore it can discharge a large dacryolith.
