Abstract Purpose: To report changes in corneal astigmatism after surgery for paralytic strabismus.

Cases and Method: This prospective study was made on consecutive series of 75 eyes of 61 cases who received surgery for paralytic strabismus in the past 17 months. The series comprised 32 males and 29 females. The age ranged from 13 to 84 years, average 59 years. There were 18 eyes with trochlear palsy, 15 eyes each with oculomotor palsy and thyroid ophthalmopathy, and 11 eyes with abducens palsy among others. Recession of medial or lateral rectus was performed on 20 eyes, resection-recession of horizontal muscles on 9 eyes, nasal transposition of inferior rectus on 19 eyes, and Nishida's procedure on 5 eyes. Corneal astigmatism was measured by automated keratometer and anterior segment optical coherence tomography. Cases were followed up for 3 months after surgery.

Results: There was no change in corneal astigmatism throughout except eyes that received resection-recession or Nishida's procedure when measured by keratometer on day 2 of surgery.

Conclusion: There was no lasting change in corneal astigmatism after surgery for paralytic strabismus.
