心病変を合併し，多発する脈絡膜肉芽腫がみられたサルコイドーシスの1例

Multiple choroidal granuloma in a case of sarcoidosis with cardiac involvement

要約　目的：肺と心病変が併発し，多発性の脈絡膜肉芽腫が両眼に生じたサルコイドーシスの1症例の報告。症例：80歳女性が9か月前からのぶどう膜炎の精査のため紹介受診した。2年前に高度の房室ブロックに対してペースメーカーの植え込みを受けていた。所見：視力は良好で，右眼に隅角の部分癒着，両眼に硝子体混濁があった。両眼の網膜と網膜下に多発する白色の結節があった。光干渉断層計で網膜内病変と脈絡膜の隆起性病変があった。全身検査でサルコイドーシスと診断された。心病変に対してプレドニゾロン内服を行い，脈絡膜と網膜の肉芽腫は3か月後に退縮した。結論：高度の房室ブロックがある高齢女性に脈絡膜の隆起性病変があるときには，サルコイドーシスの可能性がある。

Abstract.　Purpose：To report a case of sarcoidosis who showed pulmonary and heart lesions with multiple choroidal granuloma. Case：An 80-year-old female was referred to us for uveitis since 9 months before. She had received implantation of pace maker for severe atrioventricular block 2 years before. Findings：Visual acuity was 1.2 in either eye. The right eye showed slight opacity in the aqueous. Both eyes showed vitreous opacity and multiple nodules in and posterior to the retina. Optical coherence tomography showed elevated choroidal and intraretinal nodules. Systemic examinations showed pulmonary and heart lesions leading to the diagnosis of sarcoidosis. Peroral prednisolone was followed by resolution of retinal and choroidal granuloma 3 months later. Conclusion：This case illustrates that sarcoidosis may underlie in a female of advanced age in the presence of atrioventricular block and choroidal granuloma.
