Abstract 文献概要

　原発慢性閉塞隅角緑内障17眼，暗室うつむき試験で陽性となった狭隅角眼12眼，そして急性閉塞隅角緑内障の僚眼1眼，の計30眼に，アルゴンレーザーで30発前後の前凝固を実施したのち続いてNd：　YAGレーザーを用い1〜5mJの条件で虹彩切開を行った。術後の眼圧，角膜混濁，出血および凝固方法について検討を加えた。30眼中20眼に術前より11mmHgを越える眼圧上昇を認め，YAGレーザー凝固の場合にもこの点に注意することが必要と思われた。角膜の半部にも及ぶ後面混濁が2眼に観察された。出血は全例中10眼に発生したが，4mJ以下の条件で本法のようにアルゴンレーザーによる前処置を行えば3眼にみられただけであった。

Laser iridotomy was performed in 17 eyes with primary chronic angle-closure glaucoma and 13 eyes with impending angle-closure glaucoma. We first applied about 20 impacts of high-power argon laser and then completed the procedure by applying mode-locked YAG laser shots with the power level ranging from 1 to 5mJ. It was possible to create sufficient iridotomy in all the eyes.

An elevation of intraocular pressure occurred immediately after the procedure by 11mmHg or more in 20 eyes. Temporary cloudiness of posterior corneal surface occurred in 2 eyes. Cascade hemor-rhage from the site of impact by YAG laser occur-red in 10 eyes. Occurrence of hemorrhage was sig-nificantly less in frequency when the power output of YAG laser was set at 4mJ or more.
