Nd-YAGレーザーによる水晶体嚢切開術

Anterior and posterior lens capsulotomies by using Nd-YAG laser

　我々はpulsed Nd-YAGレーザーを用い白内障手術と関係の深い前嚢ならびに後嚢切開術を試みたのでその結果をのべる。

（1） Pulsed Nd-YAGレーザーにはQ-switchとMode-locked装置があるが，今回これら2種の装置を用い，KPE術前に前嚢切開を更に嚢外法，KPE，後房人工水晶体移植後の後発白内障切裂術を，また白内障術後における虹彩後癒着についてsynechiotomyを行った。Q-switch装置は，American Medical optics社，Mode-locked装置はMeditec社のものを使用した。

（2）症例は前嚢切開術，後嚢切開術，synechiotomyを含む216例250眼であった。

（3）これら手術に使用したenergy doseは，Q-switch装置では2mJ前後で目的を達し，Mode-lockcd装置ではエネルギー量が一定のため5mJで手術を行った。

（4）合併症として次のものを認めた。

a）眼圧上昇：前嚢切開術後31.8％に，後嚢切開術後13％にみられ，すべて一過性であった。

b）縮瞳：前嚢切開術において3.6％に縮瞳をみた。

c）人工水晶体（PC-IOL）移植術後の後嚢切開術でIOLのレンズ部にピットの形成を認めた。視力その他に影響はみられなかった。

We performed the Nd-YAG laser procedure as adjunct to cataract surgery using either the mode-locked unit (Meditec) or the Q-switched unit (AMO). The procedure was used as anterior cap-sulotomy prior to cataract surgery or as posterior capsulotomies or synechiotomy following cataract surgery. A total of 250 eyes were thus treated,

Cases of posterior capsulotomy with Nd-YAG laser recovered sufficient postoperative visual acui-ty.

A rise in intraocular pressure was the major com-plication of the laser therapy. All the complica-tions were of fleeting nature and subsided soon spontaneously.
