未熟児網膜症国際分類の評価とその問題点

Evaluation and problems of the new international classification of retinopathy of prematurity

　未熟児網膜症国際分類の成立の経過と厚生省未熟児網膜症研究班分類改正の経緯を述べ，活動期病期分類についてこの両者を対比して検討した．病期については国際分類のStage 1は厚生省新分類のStage 2に，Stage 2はStage 3初期に，Stage 3はStage 3中期および後期に，Stage4はStage 4および5にそれぞれ相当するので厚生省分類の病期を国際分類に書き替えることは容易であるが，逆は必ずしも可能ではない．

　国際分類では厚生省新分類のII型を特殊な病型と認めず，"＋"diseaseという概念を採用したが，"＋"diseaseはそのままII型と同一視することはできない．

　国際分類で最も大切でしかも有用なことは，病変の位置と範囲を予め定義された三つのzoneと時計の時刻で明確に記載することを規定したことである．これは今後未熟児網膜症診療に従事するすべての眼科医が必ず励行するようにしたいことであり，この規約に従った詳細な眼底所見の記載があれば，厚生省新分類，国際分類いずれを用いても客観的評価が可能である．

　日本では現在1984年1月より12施設が協力して未熟児網膜症のprospective studyを行っているが，1984年末までに集積された記録を厚生省新分類と国際分類の両者を用いて予備的に集計してみた．その結果厚生省新分類で記録されたデータを国際分類に置き換えることはほとんど問題はなかった．今後の本症の臨床的記録特に外国語による記載には上記の問題点を考慮しつつこの国際分類を積極的に用いるべきであると考える。

The international classification of acute retinopathy of prematurity (ROP) is reviewed in a historical per-spective and is compared with the 1982 version of the classification by the Japanese Joint Committee for the Study of ROP.

The international and Japanese classifications are basically similar so that it is easy to convert the staging from one to the other, provided the location, extent and degree of vascular lesions are recorded on the standard-ized scheme.

The concept of "+" disease in Type II retinopathy differs in the two systems. In the Japanese classification, the "+"disease indicates a stage of nontreated case of Type II retinopathy. It is stressed that the effect of treatment may not be expected unless the disease is diagnosed early and intervened before the "+" signs develop.

We tentatively evaluated a nation-wide series of cases with acute ROP using the international and Japanese classifications. No problems were encountered in converting the staging between the two systems.
