　片眼の眼底後極部に存在した先天性網膜動静脈吻合症で，中心性漿液性網脈絡膜症様の扁平な漿液性網膜剥離が黄斑部に発生した1例を経験した．症例は53歳男性で急に発生した中心暗点を訴えた．

　乳頭に近い耳上方の動脈の本幹部にソーセージ様の拡張と絞扼がみられ，耳上方と耳下方の静脈は強く拡張していた．

　螢光造影により，黄斑部周囲の3カ所に小血管を介する網膜動静脈吻合を認め，Archerの分類のI群と判断した．吻合部血管の1カ所から血管外漏出があり，それが網膜剥離の原因と判断された．

　特に積極的治療は行わなかったが，約4カ月の経過で，血管壁が自然に修復されて，血管外漏出がとまり，網膜剥離は消褪した．

We observed a 53-year-old male who presented with central scotoma of acute onset in his right eye. Funduscopy revealed flat serous detachment of the macula and three arteriovenous communi-cations. Fluorescein angiography showed dye staining of the vessel wall in one of the communi-cations. Extravasation of dye towards the subre-tinal space was observed at a site of the leaking vessel.The case was diagnosed as congenital arteriovenous communication of Group 1 according to the classification by Archer.Extravasation and the serous detachment of the macula disappeared spontaneously 4 months after onset.
