　先天性網膜動静脈吻合症（太い動静脈の直接吻合をみるArcher分類の第3群）で網膜に強い滲出性変化を伴った症例に対し，色素レーザーによる光凝固療法を行い，奏効した。

　症例は12歳男児で，1年前から右眼視力低下を訴え，眼底には著しい網膜動静脈の拡張，蛇行と後極部網膜の広汎な強い浮腫と多数の硬性白斑，滲出性網膜剥離がみられた。螢光眼底造影により，動静脈吻合が証明され，後極部の動静脈吻合部付近の異常血管瘤から，強い血管外漏出がみられた。左眼ならびに全身的には異常はなかった。

　治療として，色素レーザーの577nm黄色波長を用いて異常血管へ直接凝固を行った。

　螢光漏出が著明に軽減し，吻合血管の白線化と，眼底所見の改善を見た。治療前の浮腫の長期存在による黄斑部の変性のため，視力改善は得られなかったが，続発性網膜剥離の進行を予防できた。

A 12-year-old boy presented with gradual visual impairment in the right eye of 4 years' duration. We diagnosed the case as congenital arteriovenous anastomosis or syndrome of Wyburn-Mason. Fun-duscopy manifested, besides arteriovenous commu-nications, flat exudative retinal detachment and marked dilatation and tortuosity of retinal vessels. Only the right eye was affected with the disease.Fluorescein angiography revealed arteriovenous anastomosis with aneurysmal dilatations and marked extravasation of dye. We treated the dilat-ed aneurysmal vessels by directly applying dye laser at the wavelength of 577 nm.The treatment was effective in reducing exudative retinal detach-ment, vascular tortuosity and hard exudates around the edematous retinal areas.
