We treated a total of 81 eyes with rhegmatogenous retinal detachment and proliferative vitreoretinopathy with the use of scleral buckling procedure alone during the past 7-year period. All the eyes belonged to grade C or D of PVR. In eyes with PVR grade Cl or C2, the retina could be reattached in 97% of treated cases. In eyes with PVR grade C3 to D3, an anatomical success rate of 50% was obtained.

About one-half of eyes with vitreous hemorrhage or choroidal detachment in the preoperative period result-ed in anatomical failure. Throughout the present series, the visual acuity improved in anatomically attached eyes.

Our findings indicate that conventional scleral buckl-ing procedure is indicated for eyes with PVR Cl or C2. Vitreous surgery will have to be considered as a com-bined procedure when dealing with PVR of C3 or more.
