Fluorescein Angiography of Macular Folds Following Retinal Surgery

緒言

　網膜剥離手術後にみられる黄斑部皺襞形成については以前より報告はあるが，その成因についての定説はなく，すでに挙げられている諸説の中でその主なものとしては，一つは硝子体に原因を求めようとする硝子体説，他は網膜に原因を求める網膜説があげられる。

　硝子体説は硝子体の変化を重視する考えで，後部硝子体剥離が発生する時，なんらかの理由で黄斑部に硝子体剥離が起こらなかつたとき，眼球運動にさいし黄斑部に牽引力が加り，黄斑部に浮腫を生じ，黄斑部の硝子体にcondensation，網膜表面で硝子体細胞の増殖がおこる結果，黄斑部皺襞が形成されると説明するもので，Jaffe^1），Ta—nenbaum et al，^2），Lincoff^3）等がこれに属する。一方，Klien^4），Davis^5），Straatsma and Allen^6）Wise et al．^7）等は網膜の変化を重視し，preretinalmembraneを網膜起源であると述べ，黄斑部に硝子体癒着を認めず，網膜細胞要素，グリア細胞の増殖が内隣接する硝子体のcondensationにそつて延びると説明している。Wise et al．は網膜血管の障害が原因になつていると述べている。いずれにせよ，現在の段階では，生体顕微鏡によつても微細な黄斑部網膜硝子体癒着の存在を確かめることは困難で，諸説を生むもとにもなつている。

Eleven cases of macular folds following de-tachment surgery were studied by fluorescein angiography. The surgical procedure involved diathermy and scleral buckling using silicone sponge accompanied by, occasionally, cryoa-pplication or photocoagulation.

Six cases with typical macular pucker sho-wed various begrees of dye leakage from retinal capillaries in both active and inactive stages.

Out of the remaining five cases, one case presented fixed macular folds and the other four cases showed finely radiated macular folds. In these five cases, no dye leakage was obser-ved from retinal vessels.
