硝子体手術後の発症例を含む最近14年間の交感性眼炎

A 14-year-review of sympathetic ophtlialinitis, including a case after vitrectomy

　1968年から1981年までの14年間で，新患総数約11万例にたいし7例の交感性眼炎を経験した。男女比は6：1で，患者の年齢は9歳から64歳（平均36.7歳）におよび，外傷が誘因となったもの4例，手術後に発症したものが3例であった。誘因から発症までの日数は20日から90日（平均47.6日）におよんだ。

　初発病変は後極部剥離型が5例，乳頭周囲浮腫型が1例で，他の1例は受診時すでに遷延化していた。眼外症状は5例にみられた。全例が軽度ないしは高度の夕焼状眼底を呈するに至り，遷延例は脈絡膜の色素脱出が著明であった。視力は被交感眼については全例が発病前の状態まで回復したが，起交感眼は6例で0.2〜S．1．と著明な低下を遺した。発病前のステロイド投与はプレドニゾロン換算325mgに達したものが1例あったが発病を阻止できず，発病後は700〜1,700mgを1ヵ月間に投与されており，再発回数は遷延例をのぞいて0〜3回にとどまった。1例のみが起交感眼の摘出をうけたが，この手段は他例の治療成績からみて採るべきではないと考えられた。

　顕微鏡手術の導入により穿孔性外傷にたいする手術手技が向上し，外傷による発病頻度は減少する一方，新しい手術術式の確立により種々の眼内手術後に本症が惹起される可能性のあることは銘記されるべきであり，その例として後部硝子体切除術後に発症した症例を示した。

We observed a total of 7 cases with sympathetic ophthalmitis in a consecutive series of 110,000 out-patients during the 14-year-period (1968-1981). There were 6 males and 1 female. The ages of the patients ranged from 9 to 64 years. Sympathetic ophthalmitis was induced by accidental trauma in 4 and apparently by surgical manipulations in 3. The time interval between injury and onset of the disease ranged from 20 to 90 days and averaged 47.6 days.

At the initial examination, posterior retinal de-tachment was present in 5 cases and swelling of the optic disc simulating papilledema in one.
