65歳男性の両眼に生じた水晶体起因性慢性肉芽腫性眼内炎を病理組織学的に検討した。
眼球を摘出した第1眼では，破嚢した水晶体を中心に典型的なzonal typeの肉芽腫性炎症を示し，水晶体嚢内摘出を行った第2眼では，水晶体後嚢の自然破嚢を伴ってマクロファージの浸潤と類上皮細胞結節形成を示し，いずれも水晶体起因性慢性肉芽腫性眼内炎であることが明らかにされた。
本症の発症には，両眼ともに水晶体の混濁と水晶体嚢の破嚢が関係していることが考えられた。
水晶体の混濁を伴った原因不明の難治性ぶどう膜炎では，水晶体起因性炎症の可能性を考慮して，できるだけ早急に水晶体全摘出術を行うべきであることを強調した。
水晶体起因性炎症は病理組織学的には，肉芽腫性炎症であるか否かによって，「水晶体起因性慢性肉芽腫性眼内炎」あるいは「水晶体起因性慢性非肉芽腫性ぶどう膜炎」と呼ぶべきであり，臨床的には臨床所見だけから両者の鑑別は困難であるので，両者を一括して「水晶体起因性炎症」と呼ぶのが妥当であることを提唱した。
A case of bilateral lens-induced chronk granu-lomatous endophalmitis evidenced by histopa-thological study was reported. The patient, 65-year-old man, had suffered trauma in his left eye at the age of 29. At the age of 63 iridocyclitis with secondary glaucoma occurred in the left eye. It became blind at the age of 65, and uveitis with secondary open angle glaucoma occurred in his right eye. Since bilateral uveitis had been intracta-ble even with local and systemic corticosteroid, the blind left eye was enucleated. Histopathology revealed a typical granulomatous inflammation of zonal type in the eye.
