水晶体嚢内摘出術後における角膜内皮細胞の変化—第2報前房形成剤の差による影響

Specular microscopic studies in corneal endothelium after intracapsular lens extraction:Effect of filling materials in anterior chamber

　同一方法により水晶体嚢内摘出術を行った78名，134眼を対象とし，前房形成に用いた溶液の角膜内皮に及ぼす影響を検討した。対象を4群に分け，それぞれに生理的食塩水，空気，人工房水，Balanced Salt Solutionを前房形成剤として用い，角膜内皮細胞平均面積と角膜中央厚を経時的に測定した。

　Balanced Salt Solutionを使用した群の角膜内皮細胞平均面積増加率が観察期間を通して有意に小さかった。

　人口房水を使用した群の面積増加率は，観察期間のある時点では統計学的に有意ではなかったが，生理的食塩水，空気を使用した群より小さかった。

　角膜中央厚は4群間に有意差はみられなかった。

　内皮細胞平均面積の変化と角膜中央厚の変化の間には，有意な相関はみられなかった。

　内皮細胞面積の増加率を大きくしないために，前房水に類似の組成の溶液を，前房形成剤として用いることが望ましい。

The endothelial cell areas in the central cornea were measured from contact specular photomicro-graphs of 134 eyes prior to and 1, 3, 6, 9, 12 and 15 weeks after intracapsular lens extraction. Theaphakic eyes were divided into four groups accord-ing to the filling materials : balanced salt solu-tion, air, physiological saline and artificial aquaous humor, which were injected in the anterior cham-ber when all sutures were placed after intracapsular lens extraction.
