　老人性白内障全摘出術中に破嚢して生じる水晶体核の硝子体腔内への沈下は，白内障手術の重大な合併症で従来種々の処置が試みられている。今回，老人性白内障手術中に生じた水晶体核の硝子体腔内沈下の症例2例に対し，毛様体扁平部硝子体切除吸引術を応用して核の摘出に成功した。いずれも他院で白内障手術時の合併症として発生し，強角膜創を閉じて翌日本院へ転送され，本院にていったん消炎療法の上，6日後に本手術を行った。1例は前部，中部，後部硝子体切除吸引の上，灌流液にて核を前房内へ誘導し，白内障手術の強角膜創を開いて核を娩出した。他の1例は硝子体切除吸引ののち核の前房への誘導を試みたが成功せず，強角膜創を開いてopen skyとし，鑷子にて核を前房内へ移動させた上で娩出した。その後何ら合併症なく，1例は12カ月間，1例は9カ月間経過良好である。

　白内障術中に硝子体腔内へ核が沈下した際は，あわてることなく十分準備を整え，このように硝子体切除吸引術を施行して核の娩出を試みるのが良いと思われ，経験した2症例を紹介した。

Two cases were referred to us in whom the lens nucleus was dislocated in the vitreous cavity during cataract surgery. Surgical attempts were made to remove the dislocated lens nucleus 6 days after the initial operation. VISC X was used in both in-stances.

In the first case, we excised the total vitreous and then brought the dislocated lens nucleus into the anterior chamber by applying the irrigation pressure from the tip of VISC X. While the lens nucleus was kept in place by the tip from behind, the anterior chamber was opened and the nucleus was removed by applying pressure on the sclera.
