周辺部網膜血管形成異常と若年性網膜剥離

Developmental vascular abnomality in the peripheral retina in juvenile retinal detachment

　網膜剥離292例309眼を対象に，通常の検眼鏡的観察，細隙灯顕微鏡観察に加え螢光眼底造影を行い，網膜血管patternに注目し若年性網膜剥離の原因を検索した。

　網膜血管形成異常を伴う剥離が10歳代にピークを有し，若年者に高頻度に見られ，20歳未満の網膜剥離の24％を占めた。格子状変性由来の剥離は20歳代と40歳代にピークを持つ二峰性を示し，裂隙由来の剥離は60歳代にピークがあった。若年性網膜剥離では網膜血管形成異常を伴う剥離が高頻度にあり重要な発症要因の一つである。

　この網膜血管形成異常を伴う剥離眼では，（1）網膜血管走行の異常（網膜血管の直線化，分岐角度が鋭く異常多数分岐），（2）耳側周辺部の無血管野の存在，（3）耳側周辺部の網膜硝子体癒着，（4）嚢胞変性の合併，（5）両眼性が特徴であり，これらの所見は瘢痕期未熟児網膜症と酷似しているが未熟児の既往がなく，生下時に酸素投与も行われていなかった。これらの特徴を有する網膜血管形成異常の中には同一家系内発症が4例あった。

　以上の結果より網膜血管形成異常はその血管型，硝子体変化，網膜変化，両眼性より先天的な要因によるものと考えられる。

We evaluated the vascular pattern in the peri-pheral retina in a consecutive series of 309 eyes (292 cases) with retinal detachment seen during the 29 months ending May 1980. Fluorescein an-giography of the peripheral retina was routinely performed adjunct to funduscopic and slitlamp fundus examinations. There were 51 eyes (45 cases) of juvenile retinal detachment under 20 years of age.

We observed a peculiarly abnormal vascular pattern in the peripheral retina in 20% of eyes with juvenile retinal detachment.
