糖尿病性網膜症に対するアルゴンとクリプトンレーザー光凝固の対比

Comparison of argon and krypton laser in the treatment of diabetic retinopathy

　糖尿病性網膜症光凝固適応例37名61眼について，光凝固装置における同一のdelivery systemで，青緑色アルゴンレーザー例と赤色クリプトンレーザー例を比較した．

　同一患者で左右眼別に照射した18名36眼では，アルゴンもクリプトンも，治療効果・矯正視力に関して同等の成績を示した．治療量の光凝固では，重篤な合併症は皆無であった．

　同一眼底で異なった部位へ両レーザーを照射した19名25眼のうち，中間透光体に問題のない16眼で，同じ出力ではアルゴンの方がクリプトンよりも強く反応した．凝固斑の混濁を同じ程度にするのに，クリプトンではアルゴンより出力を増す必要があった．白内障や硝子体出血のあった9眼では，上の関係は逆転して，同じ出力でアルゴンの凝固斑はクリプトンに比べ弱い混濁を示した．

　赤色クリプトン光凝固では，選択的に網膜外層を凝固でき，中間透光体におけるある程度までの出血や白内障を青緑色アルゴンの場合より有効に透過した．

　以上から，進行した糖尿病性網膜症に対する汎網膜光凝固では，赤色クリプトンレーザー光凝固が先ず選択されるべきことが結論される．

We compared the therapeutic effect of blue-green argon and red krypton lasers in simple and proliferative diabetic retinopathy.

In 18 patients with similar degree of diabetic retinopathy in kind and intensity, one eye was treat-ed with argon and the other with krypton. The therapeutic effect was similar for both modalities when judged from fundus findings and visual acuity during the postphotocoagulation period. As a no-table difference, significantly less intense coagula-tion effect was obtained while using krypton than argon.

In another series of 9 eyes with cataract or vitre-ous hemorrhage treated with argon and krypton in different sessions, a lesser power output level was required to produce coagulation burns when using krypton than argon.

These findings indicate that red krypton laser results in similar therapeutic effects as argon when applied to diabetic retinopathy, whether simple or proliferative. Krypton appears to be more indicated in eyes with advanced cataract or opaque vitreous.
