乳頭欠損症における網膜視覚感度分布の検討

Fundus controlled perimetry in a case of disc coloboma

　片眼性の乳頭欠損症（colobomata）の1例を経験し，通常の動的量的視野計測とともに眼底視標による静的量的視野計測を施行した。その結果，視力不良で固視不安定な視神経の先天異常症例においては，眼底視標による静的視野計測がより正確に網膜の視機能を反映することが確認された。

　また，視覚感度は視神経乳頭に入る網膜神経線維束と密接に関連して検出された。このことから，乳頭欠損症における視野欠損は，全網脈絡膜の形成不全のみならず，単に網膜神経節細胞の形成不全あるいは逆行変性のみによっても規定されうる可能性のあることを推論した。

A case of unilateral bridge-coloboma of the optic disc with unusual visual field defect was presented. Fundus-controlled perimetry under infra-red illumi-nation and fundus photographs under red-free illu-mination were carried out. With fundus photo-graphs under red-free light, the nerve fiber bundles were better visualized and radiated from the upper margin of the optic disc. Fundus controlled peri-metry revealed the isometric photosensitivity of the retina to be located along nerve fiber bundles.

These results showed that the visual field defects were intimately related with the defects of the retinal ganglion cells.
