脳幹障害が疑われた3例で回旋成分を含む異常眼球運動を記録検討した。眼球運動はスクレラル・サーチコイルで記録した。
3例で頻度2〜4Hz,振幅1.2〜13.2度の回旋成分が記録された。このうち1例では右方視時には時計回り,左方視時には反時計回りの回旋成分を認め,滑車神経核もしくは前庭神経核を支配する神経機構の障害が示唆された。
回旋成分の定量的波形分析により脳幹部病変の局在診断に有用な情報をもたらすものと思われた。
Objective recording and quantitative evaluation of rotatory eye movements became possible with the use of scleral search coil in a magnetic field technique. We analyzed the rotatory nystagmus in 3 cases with pontine lesions involving the medial longitudinal fasciculus, MLF.
We used Skalar's eye position meter 3000 in the recording. Two patients manifested clockwise rota-tory nystagmus of 2 to 4Hz in frequency and 1.2 to13.3 degrees in amplitude. The third case manifest-ed clockwise rotatory nystagmus on rightward gaze and counterclockwise one on leftward gaze. The quick phase of rotatory nystagmus changed along with direction of gaze, suggesting that the original lesion is located in the controlling system of trochlear or vestibular nuclei. Use of scleral search coil in magnetic field technique was thus useful in quantitative analysis of rotatory nystag-mus of pontine origin.
