錐体電位による原発性黄斑部変性の分析

Electroretinographic cone potential in macular dystrophies

　眼底後極部に進行性の病変を呈する6症例のERGのrapid off-response，EOGのL／D，hyper-osmolarity responseとDiamox responseを検討した。

　（1） Stargardt病ないし後極部病変を伴うfundus flavimaculatusが疑われる1例2眼では上記4種のresponseは異常であった。

　（2） Bull's eyeを呈した4例8眼全例でrapidoff-responseは減弱したが，L／DとDiamoxresponseは正常範囲にあり，hyperosmolarityresponseは2例2眼を除いて正常範囲にあった。これら8眼では錐体障害が主病徴をなすと推論された。これらの症例が進行性錐体dystrophyならば，本症で錐体自体の機能異常が他覚的に検証されたことになる。

　（3） Rapid off-response，L／D,hyperosmolarityresponseとDiamox responseを併せ検討することは，主病変が視細胞または網膜色素上皮のいずれに存するかの推定の一助になることがある。

Six cases with macular dystrophy were examined by our new tests for cone receptor cell and retinal pigment epithelium. The cone activity was eval-uated by the rapid off-response in electroretino-gram which is composed mainly of the decay of the late cone receptor potential. The pigmentepithelial activity was assessed by the susceptibility of the standing potential to hypercsmolarity and to acetazolamide (hyperosmolarity and acetazol-amide responses).

All of the six patients noticed progressive impair-ment of visual acuity and color discrimination.
