　発生部位の異なる視神経膠腫3症例を経験し，各症例の治療方針を検討した。視神経膠腫に対して，いかなる治療も加えずに経過観察するという考えもあるが，我々は，一側の視神経のみに限局した症例では，開頭術により視交叉前より腫瘍を摘出した。視交叉部に進展した場合は，腫瘍摘出は積極的に行なわず，経結膜的に球後腫瘍の生検を行ない，組織診断を確定した。内分泌異常を伴い，視交叉周囲にも進展したと考えられる症例では，生検を含む試験開頭術を行ない，頭蓋咽頭腫や鞍上部germinomaなどと鑑別診断を行なつた。放射線療法は，腫瘍を積極的に摘出できない場合に施行し，経過観察を行なっている。

Three cases of optic nerve glioma are presented and management of each case is discussed.

In case 1, the tumor was located in the optic nerve and subtotal removal of tumor was carried out through fronto-temporal craniotomy. This pro-cedure enabled us to visualize the orbit and chiasm. A complete resection of tumor at the distal end was possible.

The case 2 was anterior unilateral chiasmal type of glioma which would extend slightly to the cont-ralateral chiasm, this assumption arising from the slight visual field defect in this side. A biopsy of retrobulbar tumor in the orbit seems to be a useful procedure.
