視交叉前症候群—球後視神経炎様症状を呈した髄膜腫症例

Prechiasmal Syndrome. Cases of Meningioma with Clinical Manifestations Simulating Retrobulbar Neuritis

緒言

　視交叉部近傍の脳腫瘍，ことに髄膜腫の場合，神経眼科学的には両眼の単性視神経萎縮，両耳側半盲を示し，いわゆる視交叉症候群といわれている。しかし非定型的な場合には球後視神経炎との鑑別が非常に困難なことがしばしばある。このたびわれわれは比較的急激な視力障害，中心暗点等の球後視神経炎様症状を呈した4症例の視交叉部近傍の髄膜腫症例に遭遇し，その問題点について検討し，視交叉前症候群を提唱したので報告する。

Three cases of meningioma in the sella tur-cica and anterior process and a case in the olfactory groove were examined. These cases showed clinical manifestations similar to retro-bulbar neuritis with sudden visual disturbance and central scotomas etc. Brief case presenta-tion and discussion are summarized below.

Case 1. (Tuberculum sellae-inner sphenoidal ridge meningioma) A 39-year-old female sud-denly developed unilateral visual disturbance with central scotoma. One month after, con-tralateral visual disturbance with central sco-toma occurred.
