緒　言

　鼻外法による涙嚢鼻腔吻合術Dacryocystorhi-nostomy （DCRと略す）は涙道開通手術の主流であり最も確実で優れた方法である。われわれも先に過去2年間にDCRを行つた23例を集計し，成功率91.3％の成績をおさめ，症例さえ選択すれば100％成功する手術であることを報告した^1）。このDCRは1904年にToti^2）が初めて鼻外法を行い，1920年にDupuy Dutemps ＆ Bourguet^3）が粘膜縫合を行い今日のDCRの術式を確立した。しかしDCRの技術的改善による適応範囲の拡大，他の術式との併用など検討の余地は残されている。とくに近年，眼科顕微鏡手術の導入により，粘膜縫合が綿密に容易に行えるようになり手術適応の拡大が可能となつた^1）。われわれは従来DCRの適応とならなかつた小涙嚢の症例や再手術の症例に対し，口腔粘膜移植とpluggingagentとしてシリコンスポンジを用いて行うDCR変法を行い好結果を得たので報告する。

Five instances of nasolacrimal duct blocks as-sociated with small lacrimal sac of four patients were repaired by a modified dacryocystorhinos-tomy with transplantation of the oral mucosal flap and silicone sponge as a plugging agent. In all cases, the silicone sponge implant was positioned by the method reported by Mirabile & Tucker (1965). In all cases it was impossible to anastomose the mucosa of the sac with the nasal mucosa. One piece of free oral mucosal flap was transplanted between the mucosas. All procedures were precisely performed under the operating microscope.
