脈なし病（高安，大西病）の3例—特に血管心臓造影所見並に免疫学的検索について

Report of the pulseless disease (3 Cases)

I．緒言

　明治41年（1907）高安氏により，眼底血管に特異な変化を来した1例を日眼誌上に報告し，大西，鹿児島両氏は橈骨動脈脈搏の触知不能を追加した。以来，中島実氏（1921）百々次夫氏（1939）等の報告があり，次で清水，佐野氏等（1948）に依つて研究され種々の名称で幾多報告される様になったが，その病態学的及び症候学的方面は一段と体系づけられる様になつた。特に清水新一氏は本症に対して初めて血管心臓造影法を行い連続撮影によつて心臓，大動脈肺動静脈，その他の血管系の状態を明らかにし飯沼，渋谷氏（1953）此を報告して以来此の方面の研究も次第に表れる様になつた。併しながら本態に関しては未だ尚不明で，結核説，梅毒説，結核アレルギー説，梅毒アレルギー説或は原因不明のアレルギーに基く説，膠原病説等の諸説があり帰する所がなく又適確な療法もないまま今日に至つている。茲で我々は脈なし病と思われる3例に血管心臓造影に加えて，免疫反応（沈降反応，赤血球凝集反応）の検索を試みたので報告する次第である。蓋し本症の免疫反応に関する研究は私共寡聞にして未だ此あるを知らないからである。

The first patient, 28 year old woman, presented a typical pulseless disease, who has complained of severe visual disturbance with a typical cerebral attack.

Clinical findings revealed that bilateral radial pulsations were not felt, showing an in-creased carotid sinus reflex.

By ophthalmoscopic findings, numerous anastomoses among vessels and new vessel for-mations in the eyeground were noticed. Moreover there was unilateral cataract.

Angiographic findings.

Bilateral carotid, vertebral and subclavian arteries etc. were not observed to be filled with "Urokolin M".
