高安・大西病初期眼底変化について

On the fundi in the early stages of Takayasu and Onishi's disease

Ⅰ．緒言

　従来脈なし病或は高安病〔茲にこれを高安・大西病と呼ぶ。以下同じ〕として報告された症例のうちには，其の眼底変化が本症のそれと全く違うもの，或は全く無いものすらがあつて，単に上半身動脈搏動の消失或は微弱の故を以つて，之れを高安・大西病と診断しているかにみえるものがある。最近の文献に此の傾向が特に著しい。

　然し茲に余のいう高安・大西病は，其の主症状としては常に，上半身主要動脈脈搏の消失或は微弱と同時に，眼底変化として特有なる乳頭周囲に於ける網膜動静脈花環状吻合の存在する疾患を指すものである。蓋し余の研究対象は上記の如き拡大された高安・大西病でなくて，其の発見が高安^12）の眼底変化の報告に発端した高安・大西病であつて，いわば狭義の高安・大西病であるから，特有なる眼底変化の存在を除外しては全く無意味となるし，且つ又茲から出発しなければ本症の病因は闡明出来ないと考えたからである。

The eyeground changes in the early stage of typical Takayasu and Onishi disease, lack-ing Burger disease sympoms, were compared with the eye ground changes in rabbits, experimentally produced by narrowing the ascending aorta or the main arteries which originate from the aorta.

The fundi in the early stages of Takayasu and Onishi disease change as follows :

1. The central blood vessels of the retina, especially the veins, are tortuous and dilated. These vessels became more apparently visible than normal condition.

2. Anastomosis of the central retinal artery and vein.
