網膜中心動脈血圧より見た起立性脳循環調節障害—主として起立性脳単独低血圧症について

Disorders of the regulation of the brain circulation by standing observed from central retinal blood pressure

I．緒言

　起立性循環調節障害については，BradburyおよびLambryの報告以来今日までSchellong，Nylinその他多数の研究があり，最近では治療面でもすぐれた報告がある。これは眩暈，頭痛，眼精疲労または疲労感を主訴とする疾患で，従来神経性循環無力症，脚気，貧血，メニエール症候群またはノイローゼと見なされていたものの中には意外にこの疾患が多いものである。最近は複雑化した環境のためにわが国においてもその数が増えており，重要な問題の一つとして注目されてきている。さて，起立性循環調節障害のある場合は当然脳循環調節障害を伴うことは推測されるから，診断に重要な体位変換試験には全身血圧以外に網膜中心動脈血圧の変動を測定することが脳循環の状態を観察する最も手近な方法であろう。しかし，上記の眩暈，頭痛等を主訴とするものの中には，全身血圧の起立性低下を伴わずに網膜中心動脈血圧のみに認める特殊な起立性低血圧が存在することが，最近眼科領域において注目されている。起立性脳単独低血圧（L'hypotension cerebraleorthostatique isolee）または起立性網膜単独低血圧（L'hypotension retienne orthostatiqueisolee）という名称で呼ばれるもので，注意して観察すると意外に多いといわれている。

The author measured how to vary the general and the retinal blood pressure by chan-ging the position of the body in 22 cases, which were 12 normal cases and 10 essential hypotension complaining of headache, dizziness etc.

The results were as follows :

1) The author discovered the disorders of the regulation of the brain circulation by standing without lowering of the general blood pressure (hypotension of the brain alone by standing).

2) Hypotension of the brain by standing was existed without any relationship to general blood pressure.
