特異な外傷性内頸動脈瘤の1例—（その2）脳血管撮影からの症例

Acase of specific traumatic aneurysm of internal carotid artery

I．緒言

　頭蓋内動脈瘤は稀な疾患ではなく，その頻度は，Martland^1）によれば2.2％，　Dial and Mau—rer^2）によれば0.5％，　Fearnsides^3）は0.8％と報じている。脳腫瘍は剖検例の1.8〜2.0％にみられると言われているから，脳動脈瘤が脳腫瘍に比し決して少くない事が分る。

　しかしながら，脳動脈瘤は古くから成因的に，先天性，動脈硬化性，細菌性及び梅毒性の4つに大別され，中でも先天性と言われる嚢状動脈瘤が大多数を占めて，Dandy^4）によれば，彼の経験例中，79.3％が先天性のものであったと言う。

Traumatic aneurysm of intracranial artery (not arteriovenous fistulas) is a rare compli-cation of head injury and has scarcely been mentioned in the literature.

Here, a case of gigantic traumatic aneurysm arising from the cavernous portion of in-ternal carotid artery is presented.

The case : A man of 23 years old, who had been treated as skull fracture following traffic accident, was now completely blind on his admission to our hospital.
