虹彩ルベオージスを伴つたKimmelstiel-Wilson症候群の1剖検例

Kimmelstiel-Wilson Syndrome with rubeosis iridis diabetica:Clinical and histological studies on a case

緒言

　糖尿病に対する治療が進歩して来たために糖尿病患者の寿命が延長した半面，合併症も多く見られるようになつて来た。然し各種の全身的障害に比較すれば，眼障害は割に少ないものの部に属するであろう。而もその眼障害の中でも糖尿病性網膜症，虹彩毛様体炎，糖尿病性白内障の発生に比較すれば虹彩Rubeosisは更に稀な合併症である。著者等は昭和32年より本年にかけて糖尿病患者を比較的に多く観察する機会を得たが，その中に1例の虹彩Rubeosisの患者に遭遇し，病理組織学的に検索することが出来た。文献によると虹彩Rubeosisの報告例は少なく^1），病理組織学的に検査し記録した例は，Kurz （1937）^2），Fehr-mann （1939）^3），Fralick （1945）^4），Gartner（1950）^5），本邦では良知・藤原（1955）^6）である。而も本症例は他に糖尿病性網膜症，腎にはKim-melstiel-Wilsonのhyalin nodulesを見出したので，此処に報告して各位の御参考に供したい。

A 55 years old man, who has had attacks of diabetes since about 10 years ago, recently complained of disturbance of vision, thirst and general weakness. He was found to have hypertension, albuminuria and glycosuria. Ophthalmologically both cataract and retinopa-thia diabetica were found. In course of treatment he developed glaucoma and then rubeo-sis iridis of the right eye. He died of uremia and general edema.

His eyes and kidneys were examined histologically.

Several peculiar conditions were as follows.

1. Vascularisation on the surface of the iris.
