Kimmelstiel-Wilson症候群の剖検例

Histopathologic study on a case of Kimmelstiel-Wilson's syndrome

I．まえがき

　Kimmeistiel-Wilson症候群（以下K-W症候群と略す）の本邦眼科領域における報告例は，最近まで10数例あるが，網膜および腎の両者についての剖検例は，奥田ら^1）（1954）および北野ら^2）（1954）の報告があるのみである。なお尾上ら^3）の報告例は腎のbiopsyがなされているのみで網膜変化については臨床的観察のみである。

　私はK-W症候群を呈する患者を約3年間にわたつて観察し，死後剖検をおこなつたので，主として網膜および腎について病理組織学的に検討してみた。

The 67-year-old man who showed Kimmelstiel-Wilson's syndrome after about 20 years' duration of diabetes was presented and at autopsy renal hyaline noduls were demonstrated as a specific diabetic process. With regard to the retinal lesion phlebosclerosis of the large veins was very remarkable and fibrosis of the retinal surface was found in the posterior polar region. Retinal microaneurysms, however, were demonstrated in a very small number on the flat preparations of the retina.

From such findings, the diabetic retinopathy seemed to run a clinical course from I (b) to V (b) in ophthalmoscopic pictures classified by Scott.
