Behçet氏病眼球の病理組織学的研究—5剖検例についての報告

Histo-pathological Studies on the Eye Balls of Behçet's Disease Report of Five Cases

　1937年Huluci Behçetは口腔粘膜の反復するアフタ，外陰部（男では陰茎及び睾丸皮膚，女では陰唇及び膣）の潰瘍及び反復する角膜エロジオン及び上鞏膜炎，或は結膜炎を主徴とする疾患を記載し，塗抹標本で細胞内（何れの細胞か明記していない）に基本小体を認めたことより，本症がウィールスに基因する疾患であり，臨床上稀ではないことを記載した。次で1939年及び1940年Triple symptom complex （Tri-symptomen-komplex）の名の下に再び同様の症候群を記載したが，眼症状としては追発性（schubweise）に発来する虹彩炎，出血を伴う結膜炎，視神経網膜炎があり，口腔粘膜及び外陰部の潰瘍の他に，下肢の結節性紅斑様の発疹，或はリウマチ様の疼痛があることを指摘した。以後皮膚，粘膜及び眼の症状を主徴とする此の疾患はBehget氏病（Be-hçet's Disease）或はBehcet氏症候群の名称を以て呼ばれている。

　然し此の症候群の他に，諸種臓器組織に類似の病変を呈する疾患群があり，種々の名称で呼ばれている。

Five eye balls which belonged to the patients in the various stages of Behçet's disease were examined histologically. The chief lesion exsists in the retina, presenting a typical inflammation of the blood vessels. Both arteries and veins are affected ; peri- and endar-teriitis in the former and panphlebitis in the latter. In the milder cases the vascular reaction consists of the infiltrstion with plasma cells and lymphocytes. In the severe cases, however, the polymorphonuclear infiltration become predominant and produce a necrotic lesion in the retinal tissues, bearing a striking resemblance to the suppurative tissue reaction.
