神戸医科大学眼科学教室における頭蓋咽頭腫の統計

Statistical Study on Craniopharyngioma in Our Clinic

　脳外科の進歩により，脳腫瘍の早期診断が次第に可能となつたと同時に，予後の為にもそれが極めて重要となつて来た。飜つて視交叉部附近腫瘍はその初期症状（視力障碍，頭痛）として患者が先ず眼科を訪れる事が多いので，我々眼科医の使命は益々重大となつた。

　頭蓋咽頭腫はCushing及びBaileyの報告例では，2023例中92例，Weberは304例中15例，其の他の吾が国に於ける報告も，北大精神科の脳腫瘍統計83例中1例と脳腫瘍中比較的稀なものと数えられているが，吾が教室で最近約10年間に取扱つた脳腫瘍手術例中，視交叉部附近腫瘍の開頭剔出例は75例であり此中頭蓋咽頭腫は36例の多きを数えた。此の比率は前記Cushingの統計その他の脳下垂体腺腫に対するものに比べかなり高率である。之は神経眼科の立場からみて本来脳外科を訪れるものと比率の異るのは当然であるが，斯くの如く眼科に多く見られる本症に対する診断が未だ眼科領域で不充分である許りでなく，頭蓋咽頭腫についての統計的観察は未だ文献が見られないので，ここにその統計的観察を試みた。

Craniopharyngionia occupied 48% of the tumors originating in the chiasm regions in our clinic. The tumor occurred frequently in younger patients and 9 cases of 34 were in the age group under 15 years. No sexual difference was observed. Eighty-two percent of the cases visited the clinic within 1—2 years after the onset of the symptoms.

The main symptoms were headaches and visual disturbances, and signs were obesity, amenorrhea, impotence, cease of growth and atrichosis. Twenty-four per cent of ocular symptoms were simple optic atrophy, and 76% included chocked disc and postoneuritic optic atrophy.
