Studies on Glaucoma (Chapter 13) :Studies on the Status of Function of Acetazoleamide (Part I)

I．緒論

　1954年Becker^1）が緑内障患者に利尿剤たる炭酸脱水酵素阻害剤acetazoleamide （Diamox）を用いて，眼圧下降作用のある事を見出して以来，Diamoxの臨床的応用及び研究が数多く発表されて来た。私達^2）3）も既にその効果に就いて発表せる如く，現在Diamoxの緑内障に対する卓効性は広く諸家に認められ，重要なる治療薬として使用されている次第である。

　併し勿ら，利尿剤であり，炭酸脱水酵素の阻害剤であるDiamoxが，如何なる機転で眼圧下降作用を示すかと云う点に関しては，現在に到るも尚完全に一致した見解に達していない。中でも最も広く信じられているのは，Becker，Kinsey等の説である。即ち重炭酸イオンが水先案内的役割を果すと云う房水産生に関するKinsey^4）の分泌拡散説と結びつき，Diamoxが毛様体中の炭酸脱水酵素を阻害して，重炭酸イオンの後房中えの分泌を抑制し，従つて房水産生を減少せしめ，眼圧を下降せしめるとする説である。

At the present there are different opinions about the hypotonia bulbi mechanism of aceta-zoleadime, the glaucoma treatment material. In the basic test of this time, decrement of ocular tension was perceived in regard to rabbit eye after the intravenous injection of dia-mox. As 20% on average of ocular tension decrement was perceived, 10 to 60 minutes later,we reviewed the action of diamox upon production and discharge of aqueous humor by the use of tonography. In regard to discharge (C) "facility of aqueous outflow" indicated a tende-ncy of increase, but this was not said to be significant syllogistically.
