Diamox Sequelsの緑内障眼眼圧下降効果について

The hypotensive effect of Diamox Sequels on the intraocular pressure in the glaucomatous eyes

　近年acetazolamideを始め，数々の炭酸脱水酵素阻害剤が出現し，その使用経験が数多く報告されている。私共も昨年Diurexの緑内障に対する治療効果^1）を報告した。その後更に副作用が少なく，投与回数が少くてすみ，尚長期間強力な眼圧下降作用を維持する薬剤の出現が期待されたが，今回Lederle会社がこの期待に添う新製剤Di—arnox Sequelsを発表し，私共は之を入手し試用する機会を得たので，本剤の緑内障に対する治療経験を報告する。

　Diamox Sequelsは，1錠中500mgのace—tazolamideを含有しているが，このうち375mgは粉末や結晶の形でなくて，0.25〜1.68mmの顆粒状となつている。この顆粒は，1／2時間に15〜30％が，2時間で27〜47％，4 1／2時間で45〜70％，7時間で67〜100％が吸収される様にwax-coatされている。従つて，血中濃度の極大は内服後8〜12時間に得られるといわれている。このsu—stained-release現象のため，眼圧に対する下降効果は，従来のDiamoxが投与後30〜90分で眼圧は下降し始め，1〜5時間で極大に達し，8〜12時間作用を持続するのに対し，Diamox Sequelsは内服後18〜24時間眼圧下降効果を有するといわれている。このため，投与回数の減少，副作用の減弱が期待される訳である。

The hypotensive effect of Diamox Sequels on the ocular tension in the various types of glaucoma was studied.

In eyes with the ocular tension over 50 mmHg., the dosage schedule employed was 2 tablets initially followed by one tablet within 24 hours. In eyes with the tension under 50 mmHg., the initial dosage was one tablet and the following maintenance dosis was decided according to the level of the lowered ocular tension.

Of 13 eyes of simple glaucoma, the effect of Diamox Sequels was (1+++) in 7 eyes (53.8%), (++) in one eye (7.7%), (+) in 4 eyes (32.5%) and (-) in one eye (7.7%).
