全身性エリテマトーデスにより角膜潰瘍を来した症例

A Case of Severe Corneal Involvement in Systemic Lupus Erythernatosus

緒言

　全身性エリテマトーデス（SLE）の眼症状は眼瞼，結膜，角膜，ブドウ膜，網膜，視神経などに生じる。しかしその多くは眼底の変化であり，角膜の障害についての報告は少ない^1〜10）。わが国のSLEによる眼症状の報告例を調査した吉本ら^4）によれば，過去42例の報告があり，すべてが眼底病変についての報告であったと述べている。

　このたびSLEの患者で，両眼の角膜中央部に点状の角膜上皮びらんと角膜浸潤を発生し，同剖にて穿孔した症例を経験したので報告し，若干の文献的考察を行なった。

A 34-year-old woman who was suffering from systemic lupus erythematosus showed mild blepharitis, diffuse injection of the bulbar con-junctiva and superficial diffuse edema at the central part of the both eyes.

Corneal edema developed rapidly to ulceration of the central cornea, and finally perforated on the 2nd month from the onset of the corneal involvement. Surgical treatment of covering with conjunctival flap on cornea was successful to the corneal ulcer.

Such a severe corneal involvement occurred three times on the right eye and two times on the left eye during the past four years.
